About the Role

The Manager will oversee day-to-day client intake, eligibility determination, service coordination, and follow-up for HMHB’s statewide Responsive Access Program (RAP) initiative. This role ensures that uninsured and underinsured individuals receive timely, culturally responsive access to reproductive and family planning services while addressing real-life barriers such as transportation, language access, and service navigation. 


Client Intake & Eligibility 
  • Conduct and oversee RAP client intake, eligibility screening, and verification, including payer-of-last-resort determination. 

  • Ensure timely access to services (within 10 days for routine care and 24 hours for urgent needs). 

  • Screen uninsured clients for Med-QUEST eligibility and support enrollment and follow-up. 


Care Coordination & Follow-Up 
  • Coordinate referrals to core and optional RAP services, including transportation assistance, doula care, and enhanced care navigation. 

  • Conduct post-service follow-up with clients to assess additional needs and gather feedback. 

  • Serve as a point of contact for clients entering RAP through HMHB, mobile clinics, outreach events, or partner referrals. 


Partner Coordination 
  • Work closely with partner sites to support intake workflows, referrals, and service tracking. 

  • Maintain secure shared tracking tools to ensure timely data exchange between HMHB and partners. 

  • Support partners in understanding RAP eligibility, documentation, and service coordination requirements. 

  • Conduct follow-up to ensure client needs are met. 


Data Entry & Reporting Support 
  • Enter and maintain accurate client-level data in all required tracking systems. 

  • Support monthly data reconciliation and preparation for DOH reporting. 

  • Flag data quality issues or service delays for escalation to the Director of RAP. 


Outreach & Engagement 
  • Participate in community outreach events, mobile clinics, and partner-hosted activities. 

  • Support RAP promotion through coordination with HMHB communications staffand The Parent Line. 

  • Ensure culturally responsive engagement with priority populations, including adolescents, postpartum individuals, LEP clients, and people experiencing homelessness.

Preferred Qualifications 

  • Master's degree in Public Health, Social Work, Health Administration, or a related field 

  • Minimum of three years of experience in direct client care 

  • Strong knowledge of trauma-informed care, reproductive health, and social determinants of health 

  • Experience with housing systems and community-based resource navigation 

  • Ability to manage complex cases and prioritize competing needs 

  • Comfort working across multiple systems, partners, and service settings 

  • Excellent communication, documentation, and organizational skills 

  • Commitment to cultural humility, equity, and client-centered care 

  • Strong interpersonal skills 

  • Proficient with data entry and knowledge of database maintenance (Apricot, Klara, etc.) 

  • Maintain a valid driver's license and State-mandated Auto Insurance 

  • Must be willing to travel, as needed
